Our mission is to build community and strengthen relationships for those living with a disability in Adelaide and regional areas of South Australia. By doing this, we help our client’s foster personal growth, well-being, and a sense of belonging that could otherwise be lost in their day-to-day lives.

How it Works

Interview & Custom Plan

Step one to a well-planned and executed NDIS plan is an interview with one our team.  We will work with you to collect relevant information, including personal details, NDIS participant number, medical history, disability-related information, support needs, and any existing NDIS plans or assessments.

This interview will provide us with invaluable information to help best determine which services, supports, and care will be of maximum impact to our clients. From here, we can conduct a needs assessment to determine the specific requirements and goals of every individual. Doing this from the beginning allows Divine Care SA to better create a plan that is most beneficial to everyone involved.

Needs Assessment

Based on the needs assessment, our team will then collaborate with our client, family members, and carers to develop an NDIS-approved support plan. The will outlines the goals, objectives, desired outcomes, and the types and frequency of support required. We will then prepare a service agreement that outlines the terms and conditions of the support services to be provided, including the scope of services, service fees, cancellation policies, confidentiality, and rights and responsibilities of both the client and the provider.

We believe in honesty, transparency, and confidentiality is evident at the forefront of everything we do. Creating a quality plan with clear objectives creates a great relationship for everyone involved.

Relevant Documentation

Navigating the different areas of the National Disability Insurance Scheme can feel daunting when not well versed with it or when your hands are full caring for loved ones and family members. Because of this, Divine Care SA will ensure that all our clients are correctly registered with the NDIS and compliant with the necessary regulations and quality standards. They may require access to relevant documentation, such as NDIS eligibility confirmation or previous plan details.

Our team will work closely with you to ensure that the plan we create is comprehensive and covers all the bases as required by the NDIS. We want you to feel at ease when trusting your loved ones with us, and thoroughness and compliance are some of the ways we do just that.

Budget Allocations & Goals

If an existing NDIS plan is already in place, Divine Care will coordinate with our client and their support coordinator to implement the agreed-upon supports and services. We must understand the approved supports, budget allocations, and goals outlined in the existing plan to ensure that any changes or adjustments are in the client's best interest and will adhere to NDIS regulations.

Our goals are always to provide the highest quality of care while maximising the funding for support supplied by NDIS.

Needs Assessment

Once the full scope of services has been assessed and determined, our team will take care of the coordination of services. Many of our clients work with multiple professionals, such as special education teachers, assistive technology specialists, social workers, occupational therapists, speech & language pathologists, and physical rehabilitation specialists, so this care coordination is of great importance. To ensure the best physical, mental, and emotional well-being of our clients, the Divine Care SA team is trained to coordinate, monitor, and determine what is in the best interest of our clients while making the most of the NDIS support budget.

At Divine Care SA, we can ensure our client’s calendar, services, and plan all well-balanced and effective for the highest levels of support.

Required Personnel

Once you begin your support plan with Divine Care SA, we monitor and regularly review your progress to ensure that the supports and services continue to meet your r needs and your goals. Life is dynamic, and so must your NDIS support plan.

As time passes, our clients often gain more independence and new skills that require less assistance from us or other outside providers. With other clients, their abilities can change due to aging or changes in their health, which can require more service and different levels and types of care both in and out of the home. When this happens, adjustments can be made to the support plan as required in consultation with our client and their support coordinator.
